Erectile dysfunction in Indian men under 40, what is actually going on

Why it happens younger than you think

Erectile difficulty in your twenties and thirties is far more common than clinic waiting rooms suggest. Indian men in particular present late, often years after symptoms start.

The penis is a vascular organ. Anything that quietly narrows blood vessels, poor sleep, untreated blood sugar, smoking, chronic stress, shows up here before it shows up in the heart.

The four common causes

Vascular: early metabolic syndrome, high triglycerides and insulin resistance are the single biggest driver we see in men under 40.

Psychological: performance anxiety creates a loop where one bad night predicts the next. This responds very well to treatment.

Hormonal: low testosterone, thyroid disorders and high prolactin account for a smaller but important slice.

Lifestyle: alcohol, nicotine, recreational drugs, five hours of sleep and long screen nights.

What your doctor should check

A useful first consult covers morning erections, timing of onset, alcohol and nicotine use, sleep hours, medications and mood.

Baseline bloods usually include fasting glucose or HbA1c, lipid profile, morning total testosterone, TSH and prolactin.

Treatments that actually work

PDE5 inhibitors such as sildenafil and tadalafil remain first line and are safe for most healthy men when prescribed correctly.

Daily low dose tadalafil suits men who want spontaneity rather than planning around a dose.

Medication works best alongside seven hours of sleep, reduced alcohol and three sessions of exercise a week. Most men notice a difference within four to six weeks.

When to get help

If difficulty has lasted more than three months, or if it appeared suddenly alongside fatigue or weight change, speak to a doctor rather than waiting it out.
